APPLICATION INSTRUCTIONS

After learning of a potential applicant's interest in our Society the proposer will request an application from Debra Swain in the SHIM Administrative Office (dswain@humanisminmedicine.org), or will print the application from this site. The proposer will deliver the application to the applicant and have him/her complete the application form. If the applicant has a curriculum vitae this should be returned with the application. The applicant and the proposer will sign the application form. The proposer will check the form to ensure that it is complete, and will return it to the Secretary/Treasurer.

The Secretary/Treasurer will furnish a copy of the application to the Chairman of the Membership Committee. The Membership Committee will review the application and recommend approval or rejection to the Board of Directors.

The Board of Directors at its annual meeting will review the application, consider the recommendation of the Membership Committee, and approve or disapprove the application.

The prospective member will be notified of his/her election to membership after action by the Board of Directors. In the event that the application is rejected, the proposer will be notified and asked to inform the applicant.
